HOW TO ACCESS THE SERVICE

Optimal Home Health works on referral basis. Though referrals typically come from a physician, they may also originate from any local hospital, doctor’s office, board and care facility, skilled nursing facility, patient, family, interested person or agency. A nurse or a qualified representative from Optimal will receive referral information. The patient is then signed up for various services requested by the physician. Our representative will see the patient within 24-28 hrs unless specific care is required on the same day.
